This infobase contains the forms subset of
the Laboratory Medicine v. 5.0 infobase. These forms have been designed for the
convenience of laboratory professionals, and have been field-tested in several
laboratories at the Unversity of Cincinnati and New York University.
All 117 forms from the Laboratory Medicine
infobase are included.
Forms are in English and in
Spanish.
Adobe Acrobat Reader software is included to
read and print the forms, which are made available in Folio™ and Adobe
Acrobat™(.pdf) formats.
Forms are customizable to the needs of your
laboratory via Folio or Adobe Acrobat software. If your laboratory does not have
either software program, you can still open the forms under Adobe Acrobat
Reader, then select, cut, and paste the document from the Reader to a Microsoft
Word® or Corel Wordperfect® document.
